Home Therapy Options

ESRD requires choosing between several options for therapy including renal replacement therapies such as in-center or home hemodialysis, peritoneal dialysis, and transplantation. In 2005, approximately 93% of patients with ESRD were initiated on hemodialysis as opposed to home therapy (Finkelstein, Story, and Firanek, 2008). While all renal replacement options offer life-sustaining treatment, home therapies offer the opportunity for better clinical outcomes with reduced mortality and morbidity, improved patient quality of life, and lower cost when compared to long-term hemodialysis (Neil, Guest, Wong, et al, 2009). Though research has shown that patients are more likely to choose home therapies if information is shared during earlier stages of Chronic Kidney Disease (CKD), these conversations do not occur consistently due to a number of factors including timing of referral to nephrology, physician comfort level with home therapy as a modality, and patient education.
