CROWNWeb

Centers for Medicare and Medicaid Services (CMS) is charged with the effective administration of Medicare benefits to eligible persons with ESRD.  CMS standardized the collection and reporting of ESRD program administration data.  In June 2012, CMS began collecting data directly from ESRD providers by establishing a system called CROWNWeb – the Consolidated Renal Operations in a Web-Enabled Network.  All Medicare-certified dialysis facilities, as well as Renal transplant and Veterans Health Administration (VA) facilities can submit ESRD program administrative and clinical data to CMS in CROWNWeb.

The transition to ICD-10 and 2728 form with new diagnosis codes:

The Centers for Medicare & Medicaid Services (CMS) is pleased to announce the new ICD-10-CM Code List—which contain the recommended ICD-10-CM codes to be used for Primary Cause of Renal Failure, effective October 1, 2015—is now available directly on the home page of www.MyCROWNWeb.org

The ICD-10-CM Code List serves as a reference source. CMS recommends using the ICD-10 Code Book/Manual available via https://www.cms.gov/Medicare/Coding/ICD10/2016-ICD-10-CM-and-GEMs.html
